Pediatric patients with severe cardiac dysfunction can benefit from the placement of a ventricular assist device. Unfortunatelly, the availability of pediatric mechanical devices is very limited, especially for the management of neonates, infants and small children. On Thursday, April 10 at 8:00AM EDT, Children's Hospital of Pittsburgh of UPMC will present a Pediatric Grand Rounds lecture titled "Mechanical Support for a Child's Failing Heart: Past, Present and Future." The presenter, Peter D. Wearden, MD, PhD, is the director of the Pediatric Mechanical Cardiopulmonary Support Program at Children's Hospital and an assistant professor of Surgery at the University of Pittsburgh School of Medicine. The lecture will be introduced by Victor O. Morell, MD, chief of Pediatric Cardiothoracic Surgery and associate professor of Surgery at the University of Pittsburgh School of Medicine. Dr. Wearden will discuss the current status and future developments in pediatric ventricular assist devices.

During the discussion, viewers will watch taped footage of the operation while the doctors describe the surgery. The doctors will answer questions from the live audience as well as live email questions from the Internet. SVR is a complex surgery that may be an option for patients with an enlarged heart and systolic heart failure. The surgery restores the heart to its normal shape and size and improves the ability of the heart to pump.

This is a simulation of the chaotic electrical activity during ventricular fibrillation. The multiple spiral waves drive the heart at a faster period than that of the normal sinus rhythm. This results in irregular activation of the ventricles and thereby prevents an effective mechanical contraction. Blood pressure instantaneously drops to zero, leading to death within minutes due to lack of cardiac output unless successful electrical defibrillation is performed. A deadly spiral in the heart. This image from Richard Clayton's research - which won the BHF's Reflections of Research science image competition in 2007 - shows a computer simulation of electrical activity in the heart during the onset of cardiac arrest. Instead of regular activity controlled by the pacemaker of the heart, electrical activation (shown in red) has looped around into a spiral that will fragment, resulting in lethal electrical anarchy.

The Thoratec Corporation is recalling some of its implantable ventricular assist devices (IVADs) because air leaks could develop in the driveline when the device is used outside the patient's body. Five injuries and one death have been reported as a result of this problem. These IVADs were designed to be implanted in the body. However, they have sometimes been used externally when certain patient characteristics prevent implantation. In the external position, damage can occur to the pneumatic driveline if it is bent at a sharp angle where the driveline meets the pump housing. One patient died when the driveline severed at its connection to the pump. The recalled devices, which were distributed to 87 hospitals in the U.S. and abroad, have serial numbers 488 or higher. The serial numbers can be found on the sterile package and on the Y-connector at the end of the percutaneous driveline. Thoratec is recommending that these IVADs not be placed in the external position. Patients who already have them in this position should be instructed to contact their doctors immediately.
